HIAWATHA FIRST NATION JOB POSTING FAMILY WELLBEING COORDINATOR TWO (2) YEAR - TERM CONTRACT To March 31, 2019 PROGRAM: Health & Social Services REPORTS TO: Health Manager WAGE RANGE: $60,000.00 - $70,000.00 TERMS OF EMPLOYMENT: TERM CONTRACT–Two Year– to March 31, 2019 ROLE: The Family Wellbeing Coordinator will work in a confidential environment and be the primary contact for the Hiawatha First Nation citizenship, who are, or may be experiencing mental health issues or are in crisis. Activities may include, but are not limited to: crisis assessments and crisis interventions, short term follow up, support, guidance and counselling individuals, families and groups in crisis that affect their mental health and wellbeing. MAIN RESPONSIBILITIES : •To establish a Family Wellbeing Model and a Crisis Intervention Team that is community and culturally based on the four quadrants of: mind; spirit; physical and emotional well being. •To provide a wholistic approach for crisis assessments and crisis interventions that involve individuals who may be experiencing crisis affect ing their well being. •To provide crisis assessments and crisis intervention supports to individuals in/or experiencing mental health crisis. •To provide community based, culturally appropriate support services to individuals, families and groups experiencing crisis with mental health or socio-economic. •To advocate and make appropriate referrals for needed client services and supports. •To be the liaison with all internal/external support plan partner services and agencies. •To complete assessments of needs for at-risk individuals pertaining to the individuals mental health issues. •To formulate and develop support plans based on the well being of the clients. •To provide case management support and services with L.I.F.E. Services team. •To maintain complete accurate confidential client documentation according to the Personal Health Information Privacy Act (PHIPA). SPECIALIZED SKILLS/REQUIREMENTS: •Mental Health First Aid Training •Non-Violent Crisis Intervention (NVCI) •Applied Suicide Intervention Skills Training (ASIST) •Ontario Common Assessment of Need (OCAN) •First Aid/CPR Level C •Excellent interpersonal skills. •Working knowledge of data base computer programs. •Must provide a clear CPIC and VSS. •Valid Driver’s License and own transportation. EDUCATION & EXPERIENCE: •Completion of Post Secondary Bachelar of Arts (BA) Degree in Social Work or Health Science discipline. •Have three (3) to five (5) years demonstrated ability to provide mental health assessments, counselling and crisis intervention supports and services. •Knowledge and appreciation of mental health issues and well being within First Nation communities. •Excellent counselling, advocating, consultation, facilitation and communication both oral and written skills. •Exceptional assessment and decision-making skills. •Proven ability to demonstrate sound clinical judgement and respect to complex client situation. •Ability to complete accurate and timely documentation. •Ability to demonstrate and maintain a professional attitude, rational detachment and a problem-solving therapeutic approach, while working with sometimes challenging clients and in stressful situations. •Knowledge and expertise in crisis intervention. •Ability to coordinate community resources and partner agencies ie: L.I.F.E.
